The Story Itself
The story starts with a guy from a gang asking Mitsuki for help, his boss is sick and the hospital he's in is holding him hostage. Mitsuki, again and again, warns the guy she's a dubious doctor, and even if she's interesting in helping, it won't come without a price. From here, we learn that Mitsuki is a someone morally grey.
She arrives in the hospital and sees many people waiting for their turn. Not being able to stand the sight, she speedran through all the guests, diagnosing and sending them home quickly. From here, we learn that despite her...dubiousness, she's a legitimate, competent doctor.
Sadly, all the guests make her unable to do actual investigation to find the gang boss. This is where Yuki steps in. It seems the story decided to use the "generally affable" part of his character here instead of the "he's a crossdresser" part of him. Since Yuki isn't an actual doctor, Mitsuki just told him to fuck off and talk to the guests or something. He does, and he hears a rumor about a ghost from some kid in the hospital and told Mitsuki about it. They investigate, and find the gang boss.
Honestly, his contribution here is "just" talking. Most girls in the Priconne are generally friendly and most (shoutout to Aoi for making me use the word most here) can talk. It's his motive sticking around the hospital that clicked for me. He just wanted to wear the cute nurse outfit. Pretty iffy, but it does make sense considering who is he.
Last but not least, Mitsuki is pretty snarky around Yuki throughout the whole story. My favorite bit is this one:
She's saying that Yuki needs a head check-up.
Difficulty
Another 100% (1000/1000) comprehension! Hey, that's a round number! Also, this count doesn't include the chibi part of the story. It's one of those that just keeps going by itself, too fast to count. I did read it, but didn't bother mining from it.
About the the hospital infiltration. Mitsuki decided to take the identity of some nurse named Naomi who's actually on vacation. It worked for the NPC nurse, but the guests and the director immediately see through her.
The nurse only realized it the second time. Why did the NPC nurse not recognize Naomi at first? Maybe she's new? Or just retarded. Either I missed the missed the explanation or it wasn't explained at all.
Translation Differences
The german loanword カルテ (karte) just went poof in the EN version. Karte is a patient's record or patient's card.
Illegal (イリーガル) is replaced with illicit. They literally mean the same thing, just makes me wonder why they bothered changing it.
The word incident and welcome aren't in the JP version at all.
An accurate translation would be something like "Illegal Oedo Karte - Dr. Mitsuki's Clinic".
Main Story Interludes
Roland has been chasing the group for a while now, hasn't he? Getting tased and gassed weren't enough to stop him. All thanks to his training, according to him. I guess that training is also why he's built like a fucking tank? Probably, but I think there's a narrative reason why he's built like that. No, it's not fanservice, this isn't GBF. What I mean by that is Roland's design is used to communicate to the audience "none of the priconnes would win against him". Even those with formal combat training like Tomo will probably...definitely lose from the sheer weight class difference alone.
Speaking of Tomo, she appeared alongside Kaya and Jun. And guess what they did? They definitely didn't fight him. They only took care the little guys while Roland was clinging to Kokoro's bike. How did the group deal with Roland? They didn't, he (and Kokoro) literally got fucking shot.
My point is that the IRL sections of the story are weirdly grounded. A massive juggernaut is coming for your ass? Nothing a gunshot or two can't fix. That is, if you ignore the espers, the existence of the mimi device, the cyborgs...okay, not grounded at all, huh.
Also, this entire situation reminds me of this shitpost.
Except the guy on the bottom wins because someone accidentally shot the big guy.
Moving on to Mitsuki, as you may have already known, she ratted the group out, and got tackled by Roland. Her motive of doing so is still unexplained, but she did a similar thing in the Illegal Clinic event.
She temporarily joined up with the director to help him finish his research for a potion that turns him into a monster. When Yuki and Yuuki confront them about it, it turns out Mitsuki had been secretly recording him for evidence to turn him in to Nightmare.
Sounds familiar, no? I think Mitsuki may have had a plan when she ratted the group out, which she couldn't execute because she got tackled. In the event, something went wrong too, the director got pissed and took more of potion. Of course, they just fought it out, since it was in-game. In-game, she's the One-Eyed Devil, but in real life, she's just an overworked woman.

Wait, Mitsuki was clearly the main character of the event. Why did Yuki sing the ED song? Goddamn narcissist.
